FEE SCHEDULE FILES
The fee schedule files are presented by state. These files contain codes from the TriSpan FISS HCPCS file which are active and contain a non-zero value in the fee amount, rehab rate or professional rate. These files contain updates through the latest updates implemented in the FISS HCPCS files. There may be some delay from the time updates are finalized and applied to the FISS HCPCS file and the updates are reflected in the files published here.

| The 1st field is the Carrier number | The 2nd field is the Locality number | The 3rd field is the HCPCS number | The 4th field is the Fee Amount (for dollar amounts the 3 far right characters are decimals.) | The 5th field is the Fee Amount (this amount is different than the first where appropriate, e.g. lab codes) | The 6th field is the Effective Date | The 7th field is the *Rehab Rate | The 8th field is the *Professional Rate |
Below are the fee files by state. To download a file, click on the file name
with your right your mouse button (right-click). This will bring up a menu that
allows you to save the file to your local computer. In Netscape, click the
"Save link as" link. In Internet Explorer, click the "Save target
as" link.
